This artwork depicts Maephisto, one of the Five Heresiarchs (five Elder Gods of Hell) from my Doom fanfic "Day of Wrath."
Maephisto (sometimes stylized Mæphisto) represents the insanity of Hell. The senselessness, irrationality, and untruthfulness.
A Heresiarch with relatively little reach, his followers are astute and cunning, as likely to build advanced Hell civilizations as they are to fall into madness in their quest for forbidden knowledge. Maephisto's followers are wizened sorcerers and ingenious scientists, and their services are often employed though with considerable mistrust from other demons.
